Multi-purpose Health cards have been sanctioned for State Government employees who receive salary by 010 account through Treasury Department will be given. According to the Medical Reimbursement which is already sanctioned for these employees, instead of cash, medical services will be provided. As first phase, this cards will be given to employees of Treasury Department. In the second phase, it will be given to State Government Employees. This procedure was commenced in district for the first time. Joint Convener of Treasury Department, Mr. K.Paleswarao released the details here on Wednesday in a press note. The forms of health card are available in the website www.treasury.ap.gov.in , he added.  He also advised that employee need to fill the form along with his family members details. Digital passport size photo of each and every family member must be submitted along with the form, he added. Filled forms can be submitted after drawing Salary to acquittance DDO officers, he added. He also stated that later the filled applications will be available online. DDO's will pass this forms to District Treasury Office. Employees need to send the details before 31st of this month, he added.
